Complete Health Assessment. This survey will be found in your order confirmation email. Please fill out + a licensed provider will ensure your purchase is FSA/HSA eligible.

If eligible you will receive a Letter of Medical Necessity (LMN) within 1-2 days. Please note this survey must be completed on the day of purchase.
Submit purchase for reimbursement. Your LMN will come with a guide outlining how to submit for your reimbursement. Claims are typically paid out within 2-4 weeks.
